If you have lost or misplaced your proximity access card that provides building and door access, use the "Start a Conversation" link on this site to submit a ticket to have it disabled and replaced.
Please see the reminders below regarding FBRI (and Carilion) proximity cards:
- Cards should be assigned only to individuals (with the exception of guest badges, see below)
- Each individual should have only one card in their possession. Additional cards should be returned to IT, HR, or FBRI Reception.
- Cards should not be shared or lent to anyone.
- If a card is lost you should:
- Report it to the FBRI Help Center (https://help.vtc.vt.edu) as soon as it is determined missing. We will disable it and request a new one for you.
- Visit the FBRI reception team to sign for a temporary guest badge.
- If you find your original card after being issued a replacement, return the found card to IT, HR, or FBRI Reception and they will deliver it to IT.
- The same above applies if you find a stray card.
- When someone leaves the institute:
- Ensure HR is aware of their departure.
- Complete an access request form to notify IT the person has left, or will be leaving on xx/xx day.
- Return the proximity card to IT, HR, or FBRI Reception.
- Cards should not be passed on to another employee or new hire.
- Guest Badges:
- Are available for sign-out from FBRI Reception.
- Are available for guests and for temporary use while replacement cards are being issued.
- If a guest badge is lost, it should be reported via the FBRI Help Center (https://help.vtc.vt.edu) as soon as it is determined missing.
- If a vendor or guest does not return the card, it should be reported via the FBRI Help Center (https://help.vtc.vt.edu)
