Lost Proximity Card

If you have lost or misplaced your proximity access card that provides building and door access,  use the "Start a Conversation" link on this site to submit a ticket to have it disabled and replaced.  

Please see the reminders below regarding FBRI (and Carilion) proximity cards: 

  • Cards should be assigned only to individuals (with the exception of guest badges, see below)
  • Each individual should have only one card in their possession.  Additional cards should be returned to IT, HR, or FBRI Reception.  
  • Cards should not be shared or lent to anyone.  
  • If a card is lost you should: 
    • Report it to the FBRI Help Center (https://help.vtc.vt.edu) as soon as it is determined missing.  We will disable it and request a new one for you. 
    • Visit the FBRI reception team to sign for a temporary guest badge. 
    • If you find your original card after being issued a replacement, return the found card to IT, HR, or FBRI Reception and they will deliver it to IT.
  • The same above applies if you find a stray card.
  • When someone leaves the institute:
    • Ensure HR is aware of their departure.  
    • Complete an access request form to notify IT the person has left, or will be leaving on xx/xx day. 
    • Return the proximity card to IT, HR, or FBRI Reception. 
    • Cards should not be passed on to another employee or new hire. 
  • Guest Badges:
    • Are available for sign-out from FBRI Reception. 
    • Are available for guests and for temporary use while replacement cards are being issued. 
    • If a guest badge is lost, it should be reported via the FBRI Help Center (https://help.vtc.vt.edu) as soon as it is determined missing.  
    • If a vendor or guest does not return the card, it should be reported via the FBRI Help Center (https://help.vtc.vt.edu)
