How to Request Network and Building Access

If you need to request network, systems, and building access for NEW and EXISTING employees, students, or volunteers, please use the ACCESS REQUEST FORM (ARF).

THIS FORM SHOULD ALSO BE USED FOR REMOVING ACCESS FOR A USER!

This includes access for: 

  • Building access / proximity cards
  • Network access (lab shares, project shares, remote access, cluster access, etc.)
  • Email list adds and removals
  • Equipment alert notifications (Edstrom / Pulse / Isensix)

IMPORTANT NOTES: 

  • We will only process requests for users after HR has notified us that the HR requirements have been met (see the last section of this article for the HR requirements) and an Access Request Form (ARF) has been submitted by the lab (see the next item).
  • To request access using the ARF, you must be a Primary Faculty member, department head, lab manager, or a documented delegate of any of the above.  We will not grant access or process requests for anyone requesting access for themselves. 
  • Prior to receiving an ARF, when we receive the HR notification we will create a disabled user account with an entry in our email address list and a "mail pointer" to their @vt.edu mailbox (or personal mail if they do not have one).  Once an ARF is received, we will convert them accordingly to an active user account.  This is to allow for inclusion in FBRI communications via email and for those users not needing actual network access, but need to receive communications. 

VIVARIUM ACCESS: Vivarium access is approved and controlled by the Vivarium Manager and staff.  If you are needing or requesting access to the vivarium, please complete the Online Access Request Form (new users) and they will be copied on your request.  After review, a member of the vivarium staff will contact you, or the access recipient, to share any requirements and to complete the access request.

In order for lab members to complete a facility tour they need to provide proof to the Vivarium Director via email to: vivarium-director@vtc.vt.edu that they are on an approved protocol corresponding to the rooms they need access to. They must also show that they have completed all IACUC trainings required (that can be a simple screenshot of the completion page). Once the Director has received that information they will set up a time that works best (usually Wednesday afternoons) to complete the facility orientation and tour. Once this has been completed and the lab member has no other questions or concerns, the Director will submit a request for badge access (and biometric access if the room uses the biometric fingerprinting system). IT will then reach out by email requesting that the lab member come in for fingerprinting.

[PLEASE NOTE]: The Online Access Request Form requires your @vt.edu / PID username and password to login.  


HR REQUIREMENTS:
Requesting access for new salary employee:

  1. Contact HR and ensure all their requirements have been met:  
    1. Offer Letter and Employee Information
    2. Virginia Tech PID Assignment
    3. Selective Service Registration Questionnaire (Male only)

    4. Vehicle Registration Form (FS-2)

    5. Tax forms

  2. Complete the Online Access Request Form

Requesting access for existing Virginia Tech employees or students:

  1. Contact HR to ensure they know the user will be working in the FBRI. 
    • Please provide full name, VT ID#, email, and department name. 
  2. Complete the Online Access Request Form

Requesting access for external collaborators (non-employees):

  1. Complete the Online Access Form

  2. Contact HR to ensure they know the user will be working in the FBRI. 
    • Please provide full name, VT ID#, email, and department name. 

Requesting access for volunteers:

  1.  Each volunteer must complete a FBRI Non-employee Form (below).
  2. Contact HR to ensure they know the user will be working in the FBRI. SubmitFBRI Non-employee Form (below).
  3. Volunteers under the age of 18 will need their parent or guardian to complete the Waiver of Liability for a Minor Volunteer (below).
  4. Complete the Online Access Request Form
